Box will appear on your screen for the promotion, type LETSROCKET
You will go to the form section, on Subject Case and Issue Fields type LETSROCKET
After that click Email Us

Just an FYI for anyone who also tried the Steam option, it won't work as the promotion doesn't apply, the promo code box only appears for Origin, PS4 and XB1.

EDIT: also just got the email that there are no codes left... :(
